(Mooringsport) RFP for Water Meters & Read Technology

SUMMARY
The Village of Mooringsport seeks to use ARPA funds (federal funds) to purchase and have installed new water meters with automatic read technology as is fully described in the RFP document.  The entity providing installation services shall be licensed by the Louisiana Contractors Licensing Board and must provide its License Number with its proposal.
BID DEADLINE
The deadline for the submission of proposals is 10:30 am Local Time, November 7, 2022.  Please check back on this portal for any extensions of the proposal due date.
MINORITY AND WOMEN OWNED BUSINESS PARTICIPATION
The Village of Mooringsport encourages minority and women owned enterprises to participate in this proposal process.  This solicitation has been listed with those who are known to notify minority and women owned enterprises.
BID SECURITY & INSURANCE
Bid security in the form of a bid bond (use the Louisiana standard form for public works projects) or cashier's check must be included with your bid in the amount of 5% of your price offering.  Electronic bidders are required to attach a copy of the original bid bond or cashier's check with their proposals.  After bid opening, a proposer will be notified to deliver the original bid security within 5 business days to:  Village of Mooringsport, 122 West Croom Street, Mooringsport, LA 71060.
ELECTRONIC ACCESS FEE
Unless it is waived (due to hardship) an electronic administrative fee of $500 will be paid by the winning contractor regardless of whether a bid is submitted electronically or physically.  The fee is payable to LaMATS as the administrator of the LACPC Municipal Cooperative Purchasing Commission, 6767 Perkins Road, Baton Rouge, LA 70808, upon completion of a final contract. 
PERFORMANCE & PAYMENT BOND
A contractor must be able to post a security bond to insure the completion of the project, including performance and payment of all liens due to labor and materials. Performance & Payment Bond shall be in the form and placed through an insurer in the manner required by a Louisiana Public Works Contract.  
ADEQUATE LIABILITY INSURANCE AND WORKERS COMPENSTION
A contractor will be required to show adequate liability insurance and workers compensation coverage to protect the Village of Mooringsport, including workers compensation indemnity provisions.
REQUIRED FEDERAL CONTRACT PROVISIONS AND DOCUMENTATION
Where federal grant dollars are to be spent, 2CFR requires that certain contract provisions be included in the contract and that document be submitted, including a statement of non-debarment or suspension and by checking the federal web portal:  https://www.sam.gov.  Firms that are capable should register in advance on this portal.  
The federal requirements described herein can be found under Attachments below.  Please review carefully as all applicable provisions will become a part of the final contract.
CONTRACT FORM
A contract form to be used (as may be modified) is included under Attachments. A final contract may or may not provide for delay damages.   
ADDITIONAL INSTRUCTIONS
Site visits are not necessary before submitting proposals.  The proposal instructions ask for the price of product and estimated installation charges only based on the number of meters, hardware and system training to be provided.  A proposer deemed capable of providing the desired product and installation services will be given an opportunity to reasonably revise its installation charges.
EVALUATION CRITERIA
25% of a proposal score will be based on the proposer's ability to provide the specified product or substantially similar product.
25% of a proposal score will be based on experience, ability to provide services and assistance after the sale, warranties and referenced similarly completed projects.
25% of a proposal's score will be based on delivery and favorable installation timeframe and installation plan.
25% of a proposal score will be based on price.
The Village of Mooringsport will be the sole judge of which proposal is most advantageous and meets the village's needs.
